Job Description
A case manager plays a critical role in supporting families involved in the child welfare system. This position involves providing case management services, advocacy, and coordination of resources to ensure the safety and well-being of children and their families. The case manager will collaborate with various agencies, service providers, and stakeholders to achieve positive outcomes for families.
Key Responsibilities:
- Case Management:
- Assess the needs and strengths of children and families.
- Develop, implement, and monitor individualized service plans.
- Maintain detailed and accurate case records and documentation.
- Provide ongoing support and follow-up with families.
- Assist families in accessing community resources, services, and benefits.
- Conduct home visits to assess family environments and needs.
- Advocacy:
- Advocate for the rights and needs of children and families.
- Represent the family’s perspective in meetings with service providers and agencies.
- Assist in resolving conflicts or barriers to services.
- Collaboration:
- Work closely with child welfare agencies, legal representatives, healthcare providers, and other stakeholders.
- Participate in case conferences, team meetings, and court hearings as required.
- Coordinate services and referrals to ensure comprehensive support for families.
- Professional Development:
- Stay informed about best practices and trends in child welfare.
- Participate in training and professional development opportunities.
- Maintain confidentiality and adhere to ethical standards.
Qualifications:
- Education:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
- Skills
- Strong communication and interpersonal skills.
- Ability to work independently and manage a caseload.
- Excellent organizational and time management abilities.
- Proficiency in using digital tools for remote communication and documentation.
- Licenses/Certifications:
- CPR Certified (or willing to get certified)
- Other Requirements
- Reliable internet connection.
- Willingness to travel for home visits as needed.
- Valid driver’s license and access to a reliable vehicle.
- Ability to pass a background check.
- Must be 21+.
