Case Manager

Job Description

A case manager plays a critical role in supporting families involved in the child welfare system. This position involves providing case management services, advocacy, and coordination of resources to ensure the safety and well-being of children and their families. The case manager will collaborate with various agencies, service providers, and stakeholders to achieve positive outcomes for families.

Key Responsibilities:

  1. Case Management:
    • Assess the needs and strengths of children and families.
    • Develop, implement, and monitor individualized service plans.
    • Maintain detailed and accurate case records and documentation.
    • Provide ongoing support and follow-up with families.
    • Assist families in accessing community resources, services, and benefits.
    • Conduct home visits to assess family environments and needs.
  2. Advocacy:
    • Advocate for the rights and needs of children and families.
    • Represent the family’s perspective in meetings with service providers and agencies.
    • Assist in resolving conflicts or barriers to services.
  3. Collaboration:
    • Work closely with child welfare agencies, legal representatives, healthcare providers, and other stakeholders.
    • Participate in case conferences, team meetings, and court hearings as required.
    • Coordinate services and referrals to ensure comprehensive support for families.
  4. Professional Development:
    • Stay informed about best practices and trends in child welfare.
    • Participate in training and professional development opportunities.
    • Maintain confidentiality and adhere to ethical standards.

Qualifications:

  • Education:
    • Bachelor’s degree in Social Work, Psychology, Sociology, or a related field.
  • Skills
    • Strong communication and interpersonal skills.
    • Ability to work independently and manage a caseload.
    • Excellent organizational and time management abilities.
    • Proficiency in using digital tools for remote communication and documentation.
  • Licenses/Certifications:
    • CPR Certified (or willing to get certified)
  • Other Requirements
    • Reliable internet connection.
    • Willingness to travel for home visits as needed.
    • Valid driver’s license and access to a reliable vehicle.
    • Ability to pass a background check.
    • Must be 21+.
